Your Appointment
The center is open Monday through Friday, 7 a.m. to 5 p.m. Hours are by appointment only. If you cannot keep an appointment, please give us one week’s notice, if possible, so we may better serve our patients. To reschedule, please contact your physician’s office. We realize your time is valuable also. Occasionally, emergencies cause delays and if your appointment time is affected, we will try to notify you as early as possible.
When scheduling your procedure, please notify your physician if you have an Internal Cardiac Defibrillator (ICD) because we do not perform procedures on patients who have an ICD.
Please bring a list of medications (including dosage and frequency) you are currently taking, your insurance card and a photo I.D. (Even though you’ve previously provided medication and insurance information to your physician, we still need this information. The center is separate from the practice office). Please notify us anytime this information changes so we may update your record.
Patients who receive sedation must arrange to have an adult accompany them to drive them home. You cannot use public transportation or a taxi unless you are accompanied by a responsible adult. If your driver decides to leave during your procedure, he or she must leave a phone number where they can be reached in case of emergency. If you have any questions, please give us a call.
